TMC Hospice is a local nonprofit serving the community since 1991. We are accredited by the Joint Commission, certified by Medicare and are a Level III partner of We Honor Veterans.Hospice cares for people with any terminal illness or injury after the patient, physician and family decide that aggressive curative treatment is no longer appropriate. Hospice care provides a sensitive support system that helps patients and their families cope with physical, spiritual and emotional concerns related to the terminal condition.Using an interdisciplinary group approach of physicians, nurses, social workers, home health aides, volunteers, spiritual counselors, bereavement counselors and others, we provide palliative care in the home, short-term inpatient/respite services, mobilization and coordination of ancillary services and bereavement support.When a patient is ready for comfort care, choosing hospice care means avoiding unwanted hospitalizations and medical treatment and procedures. Our Mission
Our mission at TMC Hospice is to provide an individualized program of physical, emotional, spiritual and practical care for adults and children in the last phases of a life-limiting illness or injury, with an emphasis on control of pain and other symptoms.
 
The program is reflective of a spirit and idea of caring that emphasizes comfort and dignity for the dying, making it possible for them to remain independent for as long as possible and in familiar surroundings. Hospice care is centered on the patient, family and caregiver. Hospice personnel respect and respond to the unique differences in family, caregiver lifestyle, values and wishes.
